    The medication circuit

    1. 01

      Reconcile

One list agreed at admission, transfer and discharge.

    2. 02

      Review and dispense

Pharmaceutical review, then dispensing against the prescription line.

    3. 03

      Administer

Dose recorded at the bedside, stock updated.

    Where it is used

    • Hospitals

      Ward administration and central pharmacy.

    • Home care

Administration recorded at the patient’s home.

    What it does not do

It does not change a prescription on its own: any substitution or dose change goes back to the prescriber or pharmacist for a decision.

    The six questions

    What is it?
A medication workflow covering reconciliation, pharmaceutical review, dispensing, administration and stock.
    Who is it for?
Pharmacists, pharmacy technicians and the nurses who administer treatments.
    Which problem does it solve?
The same patient has three medication lists — at admission, in the prescription and on the ward — and nobody is sure which one is current.
    How does it work?
Treatments are reconciled at admission, transfer and discharge. The pharmacist reviews new prescriptions with the record in view. Dispensing and administration are recorded against the same line, and stock moves with them.
    How is it different?
Prescription, review, dispensing and administration share one line per drug, so a discrepancy shows up as a gap instead of a second list.
    What is the proof?
Each administration is recorded with who, when, which batch and any reason for a missed or refused dose.
